Please print and fill out these forms and bring them at check-in. All information must be filled out prior to turning in on the day of the event. FORMS ARE ONLY REQUIRED FOR OVERNIGHT CAMPS/EVENTS AND NOT FOR REALITY CHECK, INC. YOUTH CHECK-IN DAY EVENTS.

A liability waiver is a legal document that is designed to protect individuals or organizations from liability for injuries or damages that may occur during a particular activity or event. This form is required for admittance and participation in any of Reality Check, Inc.'s events.
A medical form is a document used by Reality Check staff to collect important medical information about a student.
